Patient name: XYZ, age 14 years
Diagnosed with acute pyelonephritis.
Prescribe Cefalexin, PCM.
Special note: Child is taking lithium for bipolar.
Plan: Give appropriate medication
Cephalexin with Lithium increase risk of toxicity
Treatment: Co-amoxiclav 250/125 mg, 3 times a day for 7-10 days
