  1. Structure: Opening & Paraphrase

  2. Data Gathering: (3.5 to 4 minutes)

    1. Incident/prior knowledge: signs? Symptoms? Complications?
    2. Risk factors- ask PMH (MAM)
    3. Social history- who does he live with? Does he have any children? Are you next of kin? Who is your next of kin? Has he appointed anyone as lasting power of attorney? (Don’t ask in 60yo)

  3. Approach:

    1. Invite: I have an important update that i have been asked to share with you. Is that okay with you. Can I discuss with you?
      Is anyone else with you?
      Would you like to call someone before we start?
    2. Narrate: As we understand, …explain everything in chronological order…
    3. Warning shot: I’m afraid, unfortunately, I don’t have a very good news.
    4. Sorry: I’m really sorry to tell you, …(don’t use medical jargon)…
    5. If cry: Would you like to have some tissues?
      Wait… more cry…
      Would you like to have some water?
